ICAO 2025 SAFETY REPORT: ACCIDENT RATES FALL TO RECORD LOWS AS FATALITIES RISE, WITH ONE CRASH ALONE ACCOUNTING FOR 260 DEATHS

ICAO’s 2025 Safety Report documents commercial aviation’s safest year by accident rate on record, with fatal accidents falling 60% year-on-year to just four — but total fatalities rose from 296 to 387, with a single accident accounting for 260 deaths, underscoring the enduring challenge of preventing rare but catastrophic events in an industry carrying nearly five billion passengers annually.

FIVE BILLION PASSENGERS, 85 ACCIDENTS: AVIATION’S SAFEST YEAR BY RATE

 

The International Civil Aviation Organization has published its 2025 Safety Report, covering scheduled commercial air transport operations by aircraft with a maximum take-off mass above 5,700 kilograms. The headline data reflects a year of continued structural improvement in aviation safety: commercial airlines carried approximately five billion passengers on 38 million flights — both higher than in 2024 — while the total number of accidents fell from 95 to 85. The global accident rate declined from 2.6 to 2.2 accidents per million departures, a reduction of nearly 13%. ICAO Council President Toshiyuki Onuma said the results demonstrated that aviation could and must continue to become safer even as more people fly, and reflected decades of collaboration among states and industry to identify risks, learn from accidents, strengthen oversight and improve safety worldwide.

 

FOUR FATAL ACCIDENTS, 387 DEATHS: THE PARADOX OF THE CATASTROPHIC OUTLIER

 

The most striking improvement in the 2025 data is in fatal accidents — those in which at least one person loses their life. The number of fatal accidents fell from ten in 2024 to four in 2025, and the fatal accident rate declined by 60%, from 0.27 to 0.11 fatal accidents per million departures. Yet the total number of fatalities increased, from 296 in 2024 to 387 in 2025, and the fatality rate — deaths per billion passengers — rose from 65 to 78.

 

The explanation is a small number of accidents accounting for a disproportionately large share of lives lost. A single accident in 2025 resulted in 260 fatalities, heavily influencing the annual total. This pattern illustrates one of the central and enduring challenges facing modern aviation safety: the industry has become highly effective at preventing accidents in aggregate, but eliminating the rare catastrophic events that claim large numbers of lives simultaneously remains an unresolved priority. The asymmetry between accident frequency — which is falling — and maximum consequence — which a single event can make enormous — defines the shape of the contemporary aviation safety problem.

 

HIGH-RISK CATEGORIES: LOC-I, CFIT AND MID-AIR COLLISIONS

 

ICAO concentrates its global safety efforts on five Global High-Risk Categories of Occurrences: Loss of Control in Flight (LOC-I), Controlled Flight into Terrain (CFIT), Mid-Air Collision (MAC), Runway Excursion (RE) and Runway Incursion (RI). Together these five categories accounted for more than half of all fatal accidents in 2025. Loss of Control in Flight accounted for nearly one-third of fatal accidents within the high-risk categories and is identified as requiring the greatest ongoing attention. LOC-I events often develop rapidly, leaving limited opportunity for recovery, and addressing them requires sustained investment in pilot training, aircraft systems, safety management and human performance initiatives.

 

CFIT remained a significant concern despite only one recorded accident in the category in 2025 — that single event accounted for more than 10% of total fatalities, reinforcing the longstanding pattern of CFIT events being relatively infrequent but catastrophic when they occur. Mid-air collision events accounted for four accidents in 2025 and nearly 15% of total fatalities, underlining the importance of collision-avoidance technology and air traffic management as airspace density increases and new entrants including drones and advanced air mobility vehicles begin integration. Runway incursion provided a rare piece of unambiguously positive news: no runway incursion accidents in commercial air transport were recorded in 2025, reflecting the positive effect of sustained investment in airport procedures, technology and crew awareness programmes.

 

SAFETY OVERSIGHT: AVERAGE SCORE RISES BUT IMPLEMENTATION GAPS PERSIST

 

Beyond accident statistics, the Safety Report assesses the effectiveness of national civil aviation authority oversight through ICAO’s Universal Safety Oversight Audit Programme. The global average safety oversight score reached 69% in 2025, a continued improvement, with more states meeting ICAO’s target oversight level. The most persistent weaknesses identified relate to resolving identified safety issues, maintaining sufficient numbers of qualified technical personnel, and conducting effective ongoing surveillance and inspections. ICAO Secretary General Juan Carlos Salazar said data-driven findings were effective in prompting states to intensify focus where evidence showed the greatest risk, and that ICAO was committed to targeted capacity building, technical assistance and implementation support to help states achieve measurable safety improvements.
